Do You Take My Insurance?

We are out of network with all insurance companies. We choose to contract directly with our patients and avoid restrictions placed on our services by third party payers.

Why Don't You Take My Insurance?

We believe you deserve the best! Our mission is to provide the highest quality healthcare to every patient every visit. Unfortunately, insurance companies make this nearly impossible to achieve. We choose to be out of network with insurance so that every visit is performed by your Doctor of Physical Therapy, and you extract maximum value from the entire duration of your session. 

Do I Need a Physician's Referral?

No. A Physician referral or prescription is not necessary to be evaluated and treated by a doctor of Physical Therapy.

What Should I Wear For My Session?

Please wear athletic attire that exposes the body part you are being treated for. Please wear shorts if you are being treated for hip, knee, or foot. Please wear tank top or sports bra if being treated for neck, shoulder or arm.

What Do I Expect Each Visit?

Lewis Physical Therapy & Sports Rehabilitation is dedicated to personalized 1-on-1 care. Your physical therapist will begin each treatment seesion by performing any necessary techniques to help manage your pain that day. Further treatment will be targeted toward the root cause of your pain. This may include series of guided mobility drills, stabilization exercises, and strength training. At the end of each visit your therapist will provide you with a program to perform on your own until the next visit. Our mission is to instill independence in our patients. We identify the problem, help reduce your pain, and provide you with the tools to regain control of your body and enjoy life!

We accept payment via Visa, Mastercard, American Express, Discover, HSA and FSA, and cash.
